Identification Documents

In accordance with the Mobile Phone Fraudulent Use Prevention Act, we will verify the identity of the subscriber when applying to purchase a Golmova calling SIM or data SIM (with SMS). The information on the required documents must match the information you provide when applying for the service.

【Precaution】

・Some customers may be required to verify their identity even when applying for data SIM (no SMS).ｖ
・If you apply with user registration, identification verification of the user is required in addition to identification verification of the subscriber.
・In the following cases, we may not be able to accept your application.
　 ○Failure to submit or present personal identification documents
　 ○The information entered on the application screen does not match the information on the identification documents
　 ○If the image is unclear
　 ○In the event that the incompleteness of the documents is not resolved.
・After the start of Golmova service, you may be asked to submit your personal identification documents again by e-mail or other means.

Types of identification documents required (for customers of Japanese nationality)

Identification documents must have the same name, address, date of birth per month as that on the application, and must be within the validity period of the application.
※If your identification documents are not up-to-date, please prepare supporting documents as well.

【1 item required】

（1）Driver’s license
　 ※Issued by the Public Safety Commission.
　 ※Upload the back side of the driver's license if it has a statement on the back side.
（2）Driving record certificate
　 ※The date of issuance must be on the date or after April 24, 4.
　 ※Must have a verifiable photo of the applicant's face.
（3）Personal number card (My number card)
　 ※Upload the front side of the card (the side the photo can be confirmed).
　 ※If the back side image is uploaded, we will delete the data and ask you to resubmit the front.
　 ※My number notification card cannot be accepted as an identification document or a supporting document.
（4）Physical disability certificate (with address)
　 ※If the address is not listed, please be sure to upload an image of the supporting documents.
（5）Health and Welfare Handbook for the Mentally Disabled

（6）Health insurance card
　 ※For a card-type health insurance card, please make sure that the address is written on the back and upload this side as well.
　 ※For a paper health insurance card, please upload an image of the part where the dependent's name is written.

Please hide parts of the information when uploading documents

In accordance with the revision of the Health Insurance Law in October 2020, please hide "symbol," "number," "insurer number," and "QR code (if any)" (e.g., cover up when photographing the document or fill them in with image editing) before uploading the document. Please upload your documents after masking the information.
※Please hide the "symbol" and "number" information even if they are left blank.
※Please take a picture of the entire health insurance card so that the name of the insurer, issuing entity, and date of issuance can be seen.
※The position of the information on the health insurance card differs depending on each insurance card.

（7）Japanese Passport

　 ※Please make sure that your name and address are written in the "Holder's Name" column.

（8）Basic Resident Registration Card
　 ※If the change is noted on the reverse side, please be sure to upload the image of the reverse side as well.
　 ※Must have a verifiable photo of the applicant's face.

　 Upload one of the following documents together with supporting documents.

Types of identification documents required (for foreign customers)

If the subscriber/user is a foreign national, please present one of the following documents.

（9）Special Permanent Resident Certificate (Alien Registration Card)
　 ※Please upload the image of the back side of the card, regardless of whether or not it is written on the back side.

（10）Residence Card
　 Upload all of the following.
　 ・Front and back (regardless of whether or not it is written on the back side, please be sure to upload the image of the back side as well).
　 ※Please use an image of the back side of the card with the period of stay.

Supporting Documents

If the name and address on the identification documents are not up-to-date, you will need to submit supporting documents as well.
The documents must be issued within the last 3 months, and the name and address must be the same as those on the application.

（1）Utility bill/receipt (electricity, city gas, or water)
　 ※The building name and room number of the current address must be listed correctly.
　 ※Bills and statements printed from the Internet will not be accepted.
　 ※Demand letters for deferred payment of utility bills will not be accepted.

（2）Residence certificate
　 ※When obtaining a certificate of residence, please prepare one that does not include your personal number (My Number).
　 ※If your permanent domicile and personal number (My number) are included, please prepare an image with only your permanent domicile and personal number hidden.
