Corporate Contract Required Documents (Please prepare these documents before applying online)

Please prepare one of the following.
・Registration certificate (Certificate of all history matters/Certificate of all current matters)
・Seal certificate
*Please submit within 3 months from the issue date.
*Copies of corporate confirmation documents will not be accepted. Be sure to prepare the original.

Please prepare one of the following.
・Employee ID (copy)
·Business card
・Health insurance card (with name of contracting company)

In order to verify your identity based on the Mobile Phone Unauthorized Use Law, we will verify the identity of the person in charge of the contract.

Payment information
According to your preferred payment method,Subscriber's name or corporate nameYou can use any of the following payment methods.
Credit / debit card
Other payment methods such as direct debit and convenience store payment are in preparation.

Those satisfying the following conditions can be used.
・ Valid period is 10 days or more
・The name of the mobile phone company you are currently using and the name of the person who applied for this service are the same
* How to get the MNP reservation number is "MNP reservation application contact information"Please verify.
* If you are applying for family use, please apply after changing the name of the person who is contracting this service with the mobile phone company you are currently using.
* For the name of the contractor, please contact the mobile phone company you are using.

・Corporate identification document (original)
・Document confirming the enrollment of the person in charge (copy)
